My service includes, but is not limited to:
•Initial confidential meeting with you and chosen family members at a time and place convenient for all involved. This appointment can be held out of business hours, either in person or by phone.
•Ascertaining and agreeing the type of care required
•Completing documentation e.g. Request for assets assessment with Centrelink/DVA, application forms etc.
•Compiling a list of suitable facility options
•Organising of visits to selected facilities
•Liaising with the applicant’s health providers, carers and families, and with Aged Care providers
•Lodging applications with your preferred nursing home facilities
•Negotiating accommodation bonds, fees etc, to ensure the best possible financial outcome for the applicant
•If required, referring you to specialist aged care financial advisers
•Assisting you in understanding the Residential Care Service Agreement contract.
This service enables you to make an informed decision when an offer of placement in a nursing home arises.
